«The final measurement is carried out in the personal final meeting so that it can be viewed together with the person and the progress of the case management can be evaluated more easily. The goals in the areas of health, social, career and finances are shown individually in graphical form. This allows the person to recognise in a differentiated way how their situation has changed during case management. Positive and negative curves in the graph can be discussed and analysed in more detail. On the one hand, positive changes are shown and, on the other, it becomes clear what further steps are required for the client after the case management programme has been completed.»
